​​The Frisco High School Upper 90 Booster Club offers a wide variety of boxed meals to the players for selected soccer games. Participation is optional. Please print out a copy of the Meal Payment form and indicate whether or not your player would like to participate. If your player is participating please complete the Pre-Game Meal Order form to make meal selections. Please return the forms to the coach. Both forms are due by Friday, December 2, 2016.

Payment of $60 per player, $30 for each additional

player (includes banquet fee​)


FISD ONLINE FORMS  All forms must be filled out online except the Medical History and Physical Form, which needs to be printed, signed, and return to the coach. Click on the link above for further instructions and to print the Medical History and Physical Form.


IMPORTANT NOTE: Physicals may not be performed before May 1st of the calendar year to submit for the following school year.
